II. SCHEDULE OF CORRECTIONS: <br /> Violation #1,2,3: Effective immediately, all hazardous waste containers shall be labeled <br /> with the following information: <br /> a) The start date of accumulation. <br /> b) The words "Hazardous Waste". <br /> c) Composition and physical state of the waste. <br /> d) Statement or statements which will call attention to the <br /> particular hazardous properties of the waste. <br /> e) Name, address, and phone number of the waste <br /> generator. <br /> Violation #4: Within 30 days of receipt of this report, enter into and maintain <br /> agreements with local police and fire departments, hospitals, <br /> contractors, and State and local emergency response teams to <br /> coordinate emergency services. <br /> When all violations have been corrected, please send written certification to this office at <br /> the letterhead address. This certification shall contain the sample certification statement <br /> indicated on Attachment I. <br /> PHS-EHD may conduct a follow-up inspection at the facility to verify compliance. <br /> The issuance of this report of violation and schedule of correction does not preclude <br /> PHS-EHD from taking any administrative, civil, or criminal action as a result of the <br /> violations noted herein. <br /> If you have any questions or should require additional information, contact Eleanor Ratliff, <br /> Senior R.E.H.S. at (209) 468-0334. <br /> Ron Valinoti, REHS <br /> Director of Environmental Health <br /> DH/er /lm <br /> Attachment <br /> c: Alan Ito, CAL EPA, Department of Toxic Substances Control <br />
