II. SCHEDULE OF CORRECTIONS: <br /> Violation #1,2,3: Effective immediately, all hazardous waste containers shall be labeled <br /> with the following information: <br /> a) The start date of accumulation. <br /> b) The words "Hazardous Waste". <br /> c) Composition and physical state of the waste. <br /> d) Statement or statements which will call attention to the <br /> particular hazardous properties of the waste. <br /> e) Name, address, and phone number of the waste <br /> generator. <br /> Violation #4: Within 30 days of receipt of this report, enter into and maintain <br /> agreements with local police and fire departments, hospitals, <br /> contractors, and State and local emergency response teams to <br /> coordinate emergency services. <br /> When all violations have been corrected, please send written certification to this office at <br /> the letterhead address.
