Indy Electronics is operating a treatment facility without a per- mit issued by the Department. II. Schedule for Correction Violation # l.: Within 30 days receipt of this Report, dispose of all hazardous waste stored in excess of 90 days. Copies of the Uniform Hazardous Waste Manifest used to transnort tliose wastes shall be submitted to this office. Violation # 2 : By March 1, 1988 a correctly completed Bien- nial Report for hazardous wastes generated in 1987 shall be submitted to this Department. Violation # 3: Within 30 days of receipt of this Report, a Training Plan shall be completed outlining: - Emergency response and hazardous waste management procedures discussed in on-the-job or classroom training of personnel . - The names and titles of facility personnel who work in areas of hazardous waste management. - Verification that employees have received training and the date the training was completed.
