Correct immediately by implementing the practice of properly labeling UW. Minor-Notice to Comply. 84. Facility did not demonstrate when the universal waste lamps(UW) that are outside on the shelf of the maintenance storage area became a waste. A UW handler shall be able to demonstrate the length of time that the UW has been accumulated from the date it became a waste or was received. The UW handler may make this demonstration by: (1) Placing the UW in a container and marking or labeling the container with the earliest date that any UW in the container became a waste or was received-, (2) Marking or labeling the individual item of UW (e.g., each battery or thermostat) with the date it became a waste or was received; (3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a waste or was received-, (4) Maintaining an inventory system on site that identifies the earliest date that any UW in a group of items of UW or a group of containers of UW became a waste or was received-, (5) Placing the UW in a specific accumulation area and marking or labeling the area to identify the earliest date that any UW in the area became a waste or was received; or (6) Any other method which clearly demonstrates the length of time that the UW has been accumulated from the date it became a waste or was received. Since it cannot be determined how long this UW lamp has been on site, immediately dispose of it properly and submit a copy of the disposal record to the EHD by 8-14-11- Minor-Notice to Comply. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by 8-26-11. Note: CUPA class list provided. Immediately complete and submit the new owner/operator information form and submit it to the EHD.
