Manifest number 008643828JJK was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. By January 2, 2014, submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. 605. Several containers, located in the western repair shop, lacked complete hazardous waste labels, as follows: one 55-gallon black steel drum of mineral spirits had no accumulation start date, one 55-gallon black steel container of used grease lacked hazardous properties, and one 55-gallon black steel container of oily debris lacked hazardous properties. One yellow plastic container of waste absorbent located in the eastern repair shop and labeled "used absorbent" lacked all required labeling information. All hazardous waste containers shall be marked with the following information: 1. the words "Hazardous Waste" 2. name and address of generator 3. hazardous properties (flammable, corrosive, reactive, toxic) 4. physical state (liquid, solid) 5. composition (contents) 6. accumulation start date Immediately label these containers and ensure that all hazardous waste containers are marked with all the required information. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
