Manifest number 008643828JJK was found without a signed off copy from the <br /> destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the <br /> destination facility on site for three years and have them readily available for review. If the generator did <br /> not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility <br /> to which the generator's waste was submitted within 35 days of the date the waste was accepted by the <br /> initial transporter, the generator shall contact the transporter and/or the owner or operator of the <br /> designated facility to determine the status of the waste. If the generator did not receive a copy of the <br /> signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial <br /> transporter, the generator shall submit an Exception Report that includes a legible copy of the missing <br /> manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste <br /> and the results of those efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> By January 2, 2014, submit to the EHD a copy of the signed off copy of the manifest or a copy of the <br /> exception report submitted to DTSC. <br /> 605. Several containers, located in the western repair shop, lacked complete hazardous waste labels, as <br /> follows: one 55-gallon black steel drum of mineral spirits had no accumulation start date, one 55-gallon <br /> black steel container of used grease lacked hazardous properties, and one 55-gallon black steel <br /> container of oily debris lacked hazardous properties. One yellow plastic container of waste absorbent <br /> located in the eastern repair shop and labeled "used absorbent" lacked all required labeling information. <br /> All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all <br /> the required information. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Stacy Rivera (209) 468-3440 Mai leol (1_ . <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
