San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax:(209)468-3433 Web:www.siciov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Address: Date: <br /> Facility Name: 3233 Loomis Rd, STOCKTON October 09, 2014 <br /> USF REDDAWAY <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,GLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste. <br /> A drum of product motor oil and a drum of product antifreeze are stored in the hazardous materials shed. Both drums <br /> were observed with liquid accumulated in the top of the drum, and staining from liquid down the sides. Oily liquid was <br /> observed in the secondary containment for the drums and on the floor of the shed. The liquid and the contaminated <br /> absorbents and other solids were unlabeled. All hazardous waste shall be in properly labeled containers, marked with <br /> the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties(ignitable, corrosive, reactive, toxic) <br /> -physical state (liquid, solid) <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately place these wastes in properly labeled containers and ensure that all containers are marked with all the <br /> required information. Since it cannot be determined how long this waste has been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> This is a minor violation. <br /> 715 CCR 66273.39 Failed to keep complete records of each shipment of universal waste for three years. <br /> Copies of universal waste disposal records for the last three years were not found on site. Universal waste disposal <br /> records shall be retained for three years and have them readily available for review. Immediately locate a copy of all <br /> universal waste disposal receipts for the last three years, maintain them on site, and submit copies to the EHD. This is <br /> a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 9, 2014. <br /> Consent to perform the inspection, take photos and collect samples was given by Lincoln Boyd, Terminal <br /> Manager. <br /> 'A checklist was provided on October 9, 2014. The report was completed and mailed on October 10, 2014." <br /> This facility generated 0.13 tons of hazardous waste in the last calendar year (Jan-Dec 2013). <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at hftp://cers.calepa.ca.gov.
