Manifest numbers 000062903DAT(10/18/12), 000061000DAT(2/11/13), 006028529FLE(5/28/13), 006695833FLE (10/9/13), 005231744JJK(2/3/14), 000122838JJK(5/29/14), and 007245157FLE(7/31/14)were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a repeat violation, Class II. 110 CCR 66262.42 Failed to comply with uniform hazardous waste manifest exception requirements. This is a repeat violation, Class Il. 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. A drum of product motor oil and a drum of product antifreeze are stored in the hazardous materials shed. Both drums were observed with liquid accumulated in the top of the drum, and staining from liquid down the sides. Oily liquid was observed in the secondary containment for the drums and on the floor of the shed. The liquid and the contaminated absorbents and other solids were uncontained. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately place these wastes in properly closed containers and ensure all hazardous waste containers are closed when not adding or removing waste. This is a minor violation.
