Generator copies of manifest number 014051164JJK(9/24/15)was not sent to DTSC within 30 days of shipment of hazardous waste. The manifest was marked as having been mailed on 12/10/15. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either the generator state or the destination or consignment state. The generator manifest copy shall be mailed to: DTSC Generator Manifests P.O. Box 400 Sacramento, CA 95812-0400 Ensure that copies of all generator manifests are sent to DTSC within 30 days of shipping hazardous waste. This is a Class II violation. 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. -A green and white drum of used oil was observed in the pit in the shop with no lid -A 55 gallon green and white drum of used oil was observed under the filter crusher approximately 2/3 full with no lid. According to Mr. LaMagra, this drum is supposed to be emptied daily. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. This is a repeat violation, Class Il. Page 3 of 5
