Appliance Recyclers <br /> 1001 HSC 25212 Illegally disposed of materials that require special handling at an unauthorized point <br /> 1002 HSC 25211.4 Operated as an appliance recycler without a valid certificate <br /> 1003 HSC 25211.3 Failed to keep appliance recycling records according to this section <br /> Other Violations <br /> 9999 Other: See attached"Continuation-Official Inspection Report"for violation(s)not listed above. <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> COMPLAINT INSPECTION (000037321) <br /> A complaint was received of multiple trucks leaking oil on a continuous basis, hydraulic leaks not being <br /> repaired, and trucks leaking for week long durations of time. <br /> A complaint inspection was conducted on December 16, 2013 with Mr. Louie LaMagra and Mr. Todd <br /> Neinhouse. Oil staining was observed throughout the parking area of the facility, including numerous <br /> fresh, wet stains. Seven vehicles were observed on site in the parking area. Wet, oily spots were <br /> observed under these vehicles, but none of the vehicles were observed to be dripping and the spots did <br /> not appear to have come from the vehicles parked over them. According to Mr. LaMagra, the vehicles <br /> observed on site were only in temporary parking, and the site has begun assigned parking to better <br /> identify which vehicles are in need of repairs. According to Mr. Neinhouse, all leaks and issues with the <br /> vehicles are noted in a Daily Vehicle Report (DVR) and are addressed immediately. <br /> Unable to verify the details of the complaint.
