Failed to properly label containers of hazardous waste. All containers with <br /> hazardous waste must have a label with the following information: <br /> a. "Hazardous Waste" <br /> b. Name and address of the facility <br /> c. Content <br /> d. Accumulation start date <br /> e. Physical state <br /> f. Hazardous property <br /> Correct immediately for all containers with hazardous waste including but not <br /> limited to those mentioned in#19. <br /> 21. Failed to keep containers of hazardous waste closed except when adding or <br /> removing hazardous waste. The following containers with hazardous waste were <br /> not closed at the time of the inspection: <br /> a. Bucket of oily liquid around cutting equipment of Fabrication area <br /> b. Two drums of paint waste ash on the west side of the property <br /> c. Discarded drum ("No good, has a hole")with oily liquid <br /> d. Skimmer bucket from Stage I without a label <br /> e. Two 5-gallon buckets of oily liquid in machine shop without a label <br /> f. Container of oil skimmer waste in tool shop without a label <br /> Correct immediately for all containers with hazardous waste including but not <br /> limited to the items mentioned above. <br /> 27. Failed to keep signed copy for 3 years. The signed copy of manifest 2247288 <br /> could not be located at the time of the inspection. Submit a copy of the signed <br /> manifest to this office by 4/1/03. <br /> 28. Failed to determine the status of hazardous waste when manifest copy not <br /> received. If the signed copy of the manifest is not received by 35 days, then the <br /> facility must determine and document the status of the waste. Determine the <br /> status of the missing waste by 4/1/04 to do so within the appropriate amount of <br /> time in the future if signed manifests are not received. Corrected on 3/1/04 <br /> 29. Failed to file an exception report. It is the responsibility of the facility to file an <br /> exception report to the Department of Toxic Substance Control (DTSC) if <br /> manifests are not received within 45 days of shipment. Submit an exception <br /> report to DTSC by 4/1/04 and continue to do so in the future if signed manifests <br /> are not received. <br /> 64. Failed to retain on-site required certification.
