`J <br />wift. <br />WASTE MANAGEMENT <br />September 30, 2013 <br />Aris Cacapit <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue <br />Stockton, CA 9525 <br />11 <br />WASTE MANAGEMENT <br />Central Valley Waste Services <br />1333 East Turner Rd. <br />Lodi, CA. <br />RE: Response to Hazardous Waste Program Inspection conducted on September 5, 2013. <br />Dear Mr. Cacapit, <br />On September 5, 2013, the San Joaquin Environmental Health Department (SJEHD) conducted a <br />routine Hazardous Waste Program Inspection of Central Valley Waste Services located at 1333 <br />East Turner Rd. Lodi, CA. CVWS is a subsidiary of USA Waste of California, Inc. During the <br />inspection, SJEHD documented eleven alleged violations. Central Valley Waste Services has <br />subsequently investigated these alleged violations and has prepared the following response. <br />Attached are the manifests, training records, pictures and repair documentation that were <br />requested. <br />Violation code: 108 — Failed to determine if a waste is a hazardous waste <br />CVWS has designated a five -gallon bucket with a lid to contain the fines and treat them as <br />hazardous waste. Attachment A contains analytical test results dated 5/3/13 and 5/9/13 from <br />similar material collected at the Stockton Scavenger's facility along with a photo showing the <br />labeled container. <br />Violation code: 122, 123 & 124 — Failed to keep signed copy of manifests from the <br />designated facility for 3 years, Failed to determine status of hazardous waste when copy of <br />manifest signed by destination facility not received within 35 days. & failed to file an <br />exception report when copy of manifest signed by destination facility not received within 45 <br />days. <br />CVWS contacted Advanced Chemical Transport, Inc. on September 5, 2013 regarding missing <br />manifests. See Attachment B for copies of the manifests and Attachment C for manifest training <br />records <br />
