0 <br />Violation code: 126 — Consolidated manifest receipts failed to contain complete <br />information. <br />Training was conducted on September 11, 2013 (See Attachment C for records). <br />Violation code: 132 — Failed to maintain log for daily tank inspections. <br />The violations were noted for missing inspections in May and June 2012. CVWS has retrained <br />onsite personnel on March 13, 2013 and again on September 11, 2013. Attachment D contains <br />copies of the follow-up training records. <br />Violation code: 301— Facility not maintained to minimize the release of hazardous waste. <br />Forklift oil leak repaired on September 13, 2013. Repair order attached (Attachment E). <br />Maintenance Shop oil leak repair order and pictures attached (Attachment E). <br />Violation code: 601— Stored hazardous waste on site longer than 90 days without permit or <br />authorization. <br />Asbury was contacted on September 5, 2013 to pick up the used paint. Attachment F contains <br />copies of the invoice and manifest. <br />Violation code: 804 — Failed to: (1) Store UW lamps in a closed container and in a manner <br />that prevents breakage and release to the environment, or (2) immediately lean and <br />containerize any broken lamps or releases to the environment. <br />The (two) lamps were placed in to proper storage/shipping boxes on September 5, 2013. <br />Attachment D contains copies of the follow-up training records. Attachment H for UW lamp <br />storage photos. <br />Violation code: 808 — Failed to label Universal Waste to identify type of Universal Waste. <br />(A) Batteries (B) Mercury containing equipment (C) Electronic devices, (E) CRT's (F) <br />CRT glass. <br />New E -waste label installed on back wall on September 5, 2013.
