Metal fines generated from the secondary maintenance shop are being thrown into the trash without <br />first determining particle size or hazardous properties. Any person who generates a waste shall <br />determine if the waste is a hazardous waste. Immediately stop discarding metal fines in the trash, make <br />a hazardous waste determination for the metal fines, and manage it according to Title 22 hazardous <br />waste regulations. Particles 100 microns or smaller must be handled as hazardous waste if the metal is <br />determined to be a hazardous waste. Submit a statement and supporting documentation explaining how <br />this waste was managed by October 5, 2013. <br />122, 123, & 124. Manifest numbers 004901479FLE and 006085996FLE were found without a signed off <br />copy from the destination facility. An attempt to determine the status of hazardous waste for manifest <br />004901479FLE was done, but not for manifest 006085996FLE. Hazardous waste generators shall retain <br />copies of all manifests signed off by the destination facility on site for three years and have them readily <br />available for review. If the generator did not receive a copy of the manifest with the handwritten signature <br />of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the <br />date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or <br />the owner or operator of the designated facility to determine the status of the waste. If the generator did <br />not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was <br />accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible <br />copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate <br />the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE <br />EHD 22-02 Rev 04/19/12 / Page HW INSPECTION REPORT <br />
