Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By October 5, 2013, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. <br />126. Two consolidated manifests from Evergreen Environmental dated 8-8-12 and 4-18-13 were <br />completed using the EPA ID # CAL000302388 of a different business address. All copies of each receipt <br />shall contain all of the following information: (A) The name, address, identification number, contact <br />person, and telephone number of the generator, and the signature of the generator or the generator's <br />representative. (B) The date of the shipment. (C) The manifest number. (D) The volume or quantity of <br />each waste stream received, its California and RCRA waste codes, the waste stream type listed in <br />subdivision (c), and its proper shipping description, including the hazardous class and United Nations/ <br />North America (UN/UA) identification number, if applicable. (E) The name, address, and identification <br />number of the authorized facility to which the hazardous waste will be transported. (F) The transporter's <br />name, address, and identification number. (G) The driver's signature. (H) A statement, signed by the <br />generator, certifying that the generator has established a program to reduce the volume or quantity and <br />toxicity of the hazardous waste to the degree, as determined by the generator, to be economically <br />practicable. Correct immediately by ensuring that your own active EPA ID# CAL000194471 is used to <br />complete the consolidated manifests. <br />132. The facility had documentation to show that daily inspections of the hazardous waste oil tank were <br />conducted in January 2012 until present, but did not have documentation to show that the daily <br />inspections of the hazardous waste oil tank were done in May and June 2012. (a) The owner or operator <br />shall inspect, where present, at least once each operating day: (1) overfill/spill control equipment (e.g., <br />waste -feed cutoff systems, bypass systems, and drainage systems) to ensure that it is in good working <br />order; (2) the aboveground portions of the tank system, if any, to detect corrosion or releases of waste; <br />(3) data gathered from monitoring equipment and leak -detection equipment, (e.g., pressure and <br />temperature gauges, monitoring wells) to ensure that the tank system is being operated according to its <br />design; (4) the construction materials and the area immediately surrounding containment structures (e. <br />g., dikes) to detect erosion or signs of releases of hazardous waste (e.g., wet spots, dead vegetation); <br />and (5) for uncovered tanks, the level of waste in the tank, to ensure compliance with section <br />66265.194(b)(3).
