A site plan must be submitted identifying the locations <br />where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This is a repeat violation, Class II. <br />109 <br />CCR 2637 Failed to test secondary containment. <br />Secondary containment testing was last performed on June 26, 2008 and was due by June 26, 2011. Testing has not <br />been completed and is 48 months past due. Secondary containment testing is required once every 36 months. <br />Immediately schedule this test and provide 48 hours notification to the EHD. <br />This is a repeat violation, Class II. <br />111 <br />CCR 2638(a) Failed to test monitoring and leak detection equipment annually. <br />Annual monitoring system certification and leak detector testing were last performed on August 29, 2008 and was due <br />by August 29, 2009. Testing has not been completed and is 72 months past due. These tests are required once <br />every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware <br />that a follow up inspection will be charged at our current hourly rate. <br />This is a repeat violation, Class IL <br />112 <br />HSC 25284.2 Failed to test spill containers annually. <br />Annual spill container testing was last performed on August 29, 2008 and was due by August 29, 2009. Testing has <br />not been completed and is 72 months past due. This test is required once every 12 months. Immediately schedule <br />this test and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at <br />our current hourly rate. <br />This is a repeat violation, Class II. <br />115 <br />CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. <br />Notification was not provided to the EHD within 30 days of the change after Dan Mcllrath declared his release as a <br />designated operator from this facility in a letter dated June 14, 2009. You are required to provide a designated operator <br />and any changes shall be reported to the EHD within 30 days.
