This information was required to be submitted by July 26, 2014 . This <br />information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br />This is a repeat violation, Class II. <br />126 <br />HSC 25404(e)(4) Failed to report program data electronically. <br />A submission to the California Electronic Reporting System (CERS) for the underground storage tank (UST) program <br />has not been made. Beginning January 1, 2014, all businesses are required to submit all new (or any changes to <br />existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST <br />Response Plan, UST Owner/Operator Written Agreement (if applicable), UST Letter from Chief Financial Officer, and <br />Owner Statement of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure <br />to include any other relevant activities and required fields. <br />This is a minor violation. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. These records shall be maintained <br />on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) <br />monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample <br />analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br />elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain <br />all maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br />This is a repeat violation, Class II. <br />203 <br />CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br />Records of repairs, linings, and upgrades for the UST system were not found on site. These records shall be kept on <br />site for the life of the tank. Immediately locate and maintain these records on site and submit copies to the EHD. <br />This is a repeat violation, Class II. <br />204 <br />CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br />on site at all times.
