Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on August 29, 2008 and was due by August 31, 2009. Testing has not been <br />completed and is 34 months past due. These tests are required once every 12 months. Immediately <br />schedule these tests and provide 48 hours notification to the EHD. Please be aware that follow up <br />inspections will be charged at our current hourly rate. <br />113. Notification of designated operator has not been provided to the EHD within 30 days after the last <br />designated operator Dan Mcllrath declared his release from being as the designated operator for the <br />facility in a letter dated June 14, 2009. You are required to provide a designated operator and any <br />changes shall be reported to the EHD with 30 days. Immediately submit notification to the EHD <br />identifying all the designated operators for this facility. <br />118. An amended operational permit was not obtained when the monitoring system was changed. The <br />UST owner or operator shall notify the EHD within 30 days of any changes in the operation of the UST <br />system. Immediately submit a permit application to the EHD for this change . An "Amended Permit <br />Fee" applies. <br />120 & 317. This UST system was found to be illegally abandoned. No person shall abandon an UST <br />system without complying with the requirements of HSC 25298. Immediately obtain a permit from the <br />EHD to remove this UST system. A "Failure to Obtain a Permit" penalty applies. <br />201. Maintenance and monitoring records were not found on site. These records shall be kept on site <br />for at least 3 years . Immediately locate and maintain the on site and submit copies to the EHD. <br />202. Unable to determine if a copy of the UST operating permit was retained on site. A copy of the UST <br />operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain <br />a copy of the permit and retain it on site. <br />204. Unable to determine if the designated operator documented all the alarms and check the correct <br />test dates on the designated operator monthly inspection report, and visually inspected the UST system <br />for leaks and verified all employees have been trained during monthly inspections since your last <br />designated operator Dan Mcllrath resigned in June 2009. During the monthly inspection, the designated <br />operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br />response to any alarms to the monthly report, visually inspect all spill containers and UDC sumps, and <br />any tank top sump that had an alarm for which there is visually inspect all spill containers and UDC <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Jeffrey Wong (209) 468-0335 <br />Received By: <br />(( 6 /Z :M t <br />Title: <br />P4o t f <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
