This inspection verified disappearance of the 55 -gallon HW container with illegible Hazardous <br />Waste label observed in 2011 and cited on the June 23, 2011 inspection report; since the generator has <br />failed to submit the disposal manifest of this container, it is considered that the generator has disposed <br />or caused disposal of hazardous waste at an unauthorized point. It is illegal to dispose or cause <br />disposal of hazardous waste at a non -permitted facility. Immediately locate the missing hazardous <br />waste container, dispose in accordance to Title 22, and submit a copy of the disposal manif&ffi EHD. <br />302. Unable to verify internal emergency communication for employee in response to an emergency, <br />and Veeder Root is no longer providing services to the facility such as the spill control leader, as stated <br />in the facility's Hazardous Materials Management Plan (HMMP). Facilities shall be equipped with an <br />internal communications or alarm system capable of providing immediate instruction to facility personnel <br />in response to an emergency. Immediately provide to EHD evidence of communication or alert system <br />available at the facility that can provide immediate instruction to personnel in response to an emergency. <br />303. Unable to verify provision on site for calling outside help in case of emergency. The facility is not <br />manned, and VR has stated VR is no longer providing 24-hour monitoring as stated in the HMMP. The <br />primary and alternate persons assigned to taking actions during an emergency on the HMMP could not <br />be reached. Facilities shall be equipped with a device capable of summoning emergency assistance <br />from the local police or fire departments, or State or local emergency response teams. Immediately <br />provide to EHD evidence of device for calling outside help available at the facility in case of an <br />emergency. <br />304. Unable to verify availability of spill control or decontamination equipment on site. Facilities shall be <br />equipped with fire extinguishers, fire control equipment, spill control equipment, and decontamination <br />equipment. Immediately provide to EHD evidence of above maintenance available at the facility. <br />305. Unable to verify maintenance of communication or alarm system, spill control, or decontamination <br />equipment on site. Facilities shall maintain spill control or decontamination equipment to ensure its <br />proper operation in time of emergency. Immediately provide to EHD evidence of proper maintenance or <br />spill control or decontamination equipment at the facility. <br />306. Failed to provide access to communication or alarm during HW handling. Facilities shall be <br />equipped with communication or alarm device at the facility immediate accessible to personnel handling <br />HW.
