A <br />copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. <br />This is a Class II violation. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Ted Shackelford was not performed. The designated operator shall <br />train facility employees for which he or she is responsible in the proper operation and maintenance of the UST <br />system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a Class II violation. <br />301 <br />HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br />The following was noted at the time of inspection: <br />1) Stained oily pavement was noted around the diesel dispenser#10, the leak appeared to be originating from a <br />dispenser fuel filter, Ted Shackerlford tightened the filter and applied absorbent to address the spill. <br />2) The diesel direct -bury spill container failed when tested at the time of inspection, the fuel level was recorded at <br />3348 gallons. During the spill container repair inspection (SR0076670 on 1-27-2017, the diesel fuel level was <br />recorded at 5380 gallons, indicating a fuel deposition into the failed diesel spill container. <br />The underground storage tank system shall be operated to prevent unauthorized releases. Submit proof of <br />correction to the EHD. <br />This is a Class II violation. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The 91 STP sump sensor was located off the lowest point of the sump (some liquid was also observed in this sump) <br />and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to <br />detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by the <br />service technician.
