3) To all employees given a new job assignment for which <br /> training has not previously been received <br /> 4) Whenever new substances, processes, procedures or <br /> equipment are introduced to the workplace and represent a <br /> new hazard, and <br /> 5) Whenever FHEMCO is made aware of a new or previously <br /> unrecognized hazard. <br /> In accordance with this IIPP, training shall be provided by: <br /> * Construction Superintendant <br /> * A Member of the Safety Committee <br /> * Or any Officer of the Company or other designee <br /> as may be assigned. <br /> For Tailgate meetings the Foreman will conduct the meeting at least <br /> once a week on the jobsite. See Exhibit D for a copy of the <br /> meeting form to be completed. <br /> This IIPP shall be made an integral part of existing occupational <br /> safety and health training programs at FHEMCO. <br /> MAINTENANCE OF RECORDS (8 CCR 3203 (b) ) <br /> FHEMCO will keep records of the actions taken to implement and <br /> maintain this IIPP. The records will be maintained on file for a <br /> minimum of three years. The records kept by FHEMCO relating to <br /> this. IIPP will not adversely affect the retention of medical and <br /> exposure records in accordance with Title 8 , California Code <br /> of Regulations, Section 3204 "Access to Employee Exposure and <br /> Medical Records. " <br /> Records of scheduled and unscheduled periodic inspections as well <br /> as other records including methods used to identify and evaluate <br /> workplace conditions and work practices shall also be retained. <br /> Records relating to the IIPP shall include at a minimum, person(s) <br /> conducting the inspection or evaluation; the unsafe conditions and <br /> work practices that have been identified; and, actions taken to <br /> correct the identified condition or work practice. <br /> Records and documentation of safety and health training shall <br /> include at a minimum, the name of employee and/or employee number; <br /> date of training; training topic(s) ; training format; and <br /> instructor. <br /> Records of Safety Committee minutes shall also be retained for <br /> three years. <br /> 10 <br />
