d. Document the corrective action and date corrected in <br /> accordance with this Section. The documentation is to be <br /> completed by the Responsible Person or her/his designee. <br /> Documentation will be maintained on file by the <br /> Responsible Person. <br /> Unsafe or unhealthy work conditions needing corrective action will <br /> be documented by using: <br /> a. A form (Exhibit C) that is to be completed in assessing <br /> the safety conditions of a site must include the name of <br /> of the person identifying the hazards with an attached <br /> checklist noting the corrective action assignments and <br /> expected date for completion. <br /> b. Engineering controls will be used first to eliminate or <br /> minimize unsafe or unhealthy work conditions. If <br /> engineering controls are impractical or infeasible, <br /> administrative controls will be used. If engineering <br /> controls alone, or in combination with administrative <br /> controls cannot adequately minimize the hazard, personal <br /> protective equipment shall be used. <br /> c. Unsafe work practices will be immediately corrected by <br /> providing the affected employees with re-training to be <br /> provided by the Responsible Person or her/his designee. <br /> All operating Procedures will be reviewed at least annually <br /> and whenever new chemicals or equipment are introduced into the <br /> Company, or when there is a procedural change. <br /> When changes are made, affected employees will receive additional <br /> instruction. <br /> TRAINING AND INSTRUCTION (8 CCR 3203 (x) (7) ) <br /> All employees shall receive training and instruction in the <br /> following areas: <br /> 1) General safety and health work practices <br /> 2) Specific instruction with respect to hazards unique to <br /> the job assignment; <br /> Training of employees at F'HEMCO as to this IIPP shall occur: <br /> 1) When the program is first established.
