Vehicle has an audible reverse signal alarm <br /> 2 . Vehicle is backed up only when observer says it is <br /> safe to do so . <br /> 13 . CONTROL OF HAZARDOUS ENERGY . <br /> "'The following procedures are established in compliance with <br /> OSHA Rule 29 CFR 1910 . 147 which regulates the Control of <br /> Hazardous Energy . The purpose of these procedures is to <br /> prevent possible employee injury that may result from <br /> unexpected energization or start up of equipment while it is <br /> being maintained or serviced . " <br /> a . Any employee performing maintenance or servicing of <br /> equipment must ensure that the energy source ( i .e . , <br /> electrical , mechanical , hydraulic , pneumatic , chemical , <br /> thermal . etc . ) to the equipment being serviced is <br /> disconnected . <br /> b . If the energy source can be lacked out in the off <br /> position to prevent unexpected energization of the <br /> equipment , affix a lock or lockout device to the energy <br /> source . Indicate on an accompanying tag the name of your <br /> company and why the energy source to the equipment is <br /> locked . Example : "Equipment being serviced" . DO NOT <br /> TURN ON . or OUT OF ORDER . DO NOT TURN ON . DO NOT <br /> OPERATE . <br /> Note : IT IS NOT PERMISSIBLE TO USE TAGS IN PLACE OF <br /> LOCKS . If an energy source is lockable then a lock must <br /> be used to prevent energization . <br /> C . If the energy source was not designed to accept a lock , <br /> then it must be tagged out . Provide the same information <br /> on the tag as indicated on #2 above . <br /> d . After locking out the energy source and before proceeding <br /> with maintenance or servicing of the equipment , the <br /> employee must test the equipment to verify that the <br /> energy source is indeed isolated . <br />
