April 19, 1989
Mr. Sydney Levin
1800 Monterey Road
San Jose, CA 95112
RE: PERMIT APPLICATION - 1250 S. WILSON WAY; STOCKTON
FAIRGROUNDS INDUSTRIAL PARK SUBSURFACE PROPERTY EVALUATION
An incomplete permit application for the purpose of evaluating the above referenced underground
storage tank facility has been submitted to this office.
The underground storage tanks have not undergone registration with the San Joaquin Local Health
District (SJLHD). In order for our review process to continue on the subsurface property evaluation
and to enable the SJLDH to issue an operating permit, the following is required:
1. Complete the enclosed registration forms (A & B) and return to SJLHD.
2. Remit to the SJLHD the appropriate registration fees as outlined on the enclosed worksheet.
3. Quarterly Monitoring Statements shall be submitted for all tanks.
4. Daily Inventory Reconciliation shall be performed utilizing alternative #5 for all motor vehicle
fuel tanks possessing calibratable meters;
5. Those tanks containing fuel but without meters shall utilize alternative #7 providing the
monitoring criterion can be met;
6. Enclosed is an "Application For Permit to Close" packet should you decide to remove the
UGST's rather than maintain daily and annual monitoring requirements. The closure information is
then submitted to our department for review for adequacy and compliance to the Underground
Storage Tank Law, (Title 23, CCR); and an inspection is conducted to witness the closure activities.
To avoid administrative and/or legal action, all required data shall be submitted to this department
within 15 :days from the date of this letter.
If you have any questions regarding the tank registration, please contact William R. Snavely of my
staff at (209) 468-3449.
Sydney Levin
