April 19, 1989 <br /> Page 2 <br /> Regarding the subsurface property evaluation, pursuant to the SJLHD fee schedule Resolution No. <br /> 85-58, a $35.00 consultation fee must be submitted along with the completed general environmental <br /> health application form for review of the above referenced environmental assessment/remediation <br /> activity. Enclosed is an informational packet outlining the steps to obtain the necessary permits to <br /> facilitate the investigation. <br /> Each additional hour over the initial one hour of time spent in review activity will be billed at the <br /> rate of $35.00 per hour. The additional amount due will be billed to the applicant identified on the <br /> general environmental health application form at the completion of requested activity, or monthly, if <br /> longer than 30 days is required for completion. <br /> The current workload volume is being managed by a 60-day backlog of chronologically received <br /> environmental assessment/remediation reports and/or workplans. The SJLHD has established a <br /> priority review process which is an option available upon written request. The priority review is <br /> conducted during off hours at an overtime rate of $52.50 an hour. Scheduling of the priority review <br /> is accomplished as staffing permits upon receipt of written request and the submitted assessment <br /> document. <br /> Should you have any questions concerning the matter concerning the subsurface evaluation, contact <br /> Diane M.
