M • <br /> 24 API RECOMMENDED PRACTICE 1646 <br /> 7.6.2.5 High work access <br /> When using an extension ladder to gain access to a roof or other high work platform,the side rails of the ladder shall extend at <br /> least 36 inches above the level of the area being accessed <br /> 7.6.2.6 Climbing <br /> • Always face the ladder when climbing up or down. <br /> • Always maintain three-points-of-contact(one hand and two feet,or two hands and one foot)when climbing up or down. <br /> • Workers shall not carry any object or load on a ladder that may cause the employee to lose balance and fall. <br /> • Do not use the cross-bracing on the rear of a ladder for standing or climbing unless the ladder is designed and provided with <br /> steps for climbing on both front and rear. <br /> 7.6.2.7 Moving/Shifting <br /> Do not move,shift,or extend a ladder when someone is on it. <br /> 7.6.2.8 Horizontal Reach <br /> Do not overreach beyond the rail of the ladder(a person's midpoint shall not extend beyond the mil ofthe ladder) <br /> 7.6.2.9 Visual Restrictions <br /> Do not use a ladder if you are visually restricted by a blasting hood,welding helmet,and/or burning goggles. <br /> 7.6.2.10 Prohibited Ladders <br /> Do not use the following Types of ladders: <br /> • Single-rail ladders <br /> • Ladders constructed on the job site <br /> 7.7 USE OF PERSONAL PROTECTIVE EQUIPMENT AND TIE-OFF POINTS <br /> 7.7.1 Use of Fall Arrest Equipment <br /> Fall-arrest equipment is the primary personal protective equipment(PPE)required to be used for working at heights operations <br /> when the hazard cannot be eliminated by engineering controls. Working at a height where the feet or the worker are six feet or <br /> higher above the ground shall not begin unless: <br /> • A fixed platform is used with guardrails or handrails,verified by a competent person,or <br /> • fall arrest equipment is in use that has: <br /> — a proper anchor, <br /> — full body harness using double latch self locking snap hooks at each connection, <br /> — synthetic fiber lanyards, <br /> — shock absorber. <br /> • Fall arrest equipment shall limit free fall to 6 feet or less. <br /> • A visual inspection of the fall arrest equipment and system is completed and any equipment that is damaged has been taken <br /> out of service. <br /> Fall arrest requirements when working on ladders may be different for individual customers. <br /> 7.7.1.1 Personal Fall Arrest System <br /> A system used to arrest an employee in a fall from a working level.It consists of an anchorage,connectors,a body harness and <br /> may include a lanyard,deceleration device,lifeline,or suitable combinations of these. Anchorage points shall be able to with- <br /> stand at least 5000 pounds of force.Employers using personal fall arrest systems shall properly train employees and have a rescue <br /> plan for any employee who falls. Specific OSHA requirements for personal fall arrest systems may be found at 29 CFR <br /> 1926.502(a)(http:\\www.osha.gov). <br /> 7.7.1.2 Operational Requirements <br />
