36 API RECOMMENDED PRACTICE 1646 <br /> 11.5 ATMOSPHERIC TESTING INCLUDING SAFE ATMOSPHERIC LEVELS <br /> Before any person enters either a permit-required or non-permit-required confined space,the atmosphere inside the space shall be <br /> tested by a person trained in for hazardous atmosphere conditions. <br /> 11.6 VENTILATION METHODS <br /> After initial testing of the atmosphere,the space shall be ventilated with a mechanical ventilation system designed for ventilating <br /> confined spaces.The atmosphere shall be re-tested to ensure that the supply of air from the ventilation unit has not introduced any <br /> additional hazards into the space. <br /> 11.7 ASSIGNMENT DUTIES <br /> For construction sites or existing retail petroleum/convenience sites,the individual in charge of performing the work is responsi- <br /> ble for identifying all confined space areas as well as implementing and enforcing the confined space entry program. there are <br /> three general roles and responsibilities for confined space entries—the entry supervisor,authorized entrant and attendant.These <br /> individuals shall receive special training before working in or near confined spaces.No other personnel on the site shall be in the <br /> confined space work area. <br /> 11.8 ENTRY PROCEDURES FOR CONFINED SPACES <br /> Contractors who enter confined spaces shall develop and implement the means,procedures,and practices necessary for safe con- <br /> fined space entry operations,including,but not limited to the provisions of this Safe Work Practice.Such means procedures,and <br /> practices shall: <br /> • Be in full compliance with all applicable federal,state,and local regulations. <br /> • Be written as the contractor's confined-space compliance policy. <br /> • Include as a minimum pre-entry procedures,pre-entry checklist and confined space permit execution. <br /> 11.9 RESCUE PROCEDURES FOR SINGLE OR MULTI-EMPLOYER ENTRIES <br /> Rescue procedures shall be established for each confined space entry.All necessary rescue procedures shall be developed before <br /> confined space entries may begin. <br /> Workers who have not received special training for confined space and who encounter an emergency should not attempt a rescue <br /> by themselves.This may endanger the would-be rescuer as well as reduce the likelihood of rescuing the original person.Immedi- <br /> ately seek help which should include the job superintendent or person in charge.Call 911 immediately and inform them of"man <br /> down in a confined space'and the type of space. <br /> 11.10 PERSONAL PROTECTIVE EQUIPMENT <br /> For all confined space entries greater than 5 feet in depth,full retrieval gear shall be used.Additionally,site-specific rescue proce- <br /> dures and contact numbers shall be documented and in place prior to the start of work.Full retrieval gear consists of(at a mini- <br /> mum): <br /> • tripod specifically designed and certified as a personnel retrieval device, <br /> • properly sized and fitted full body harness connected to the tripod and wom by the entrant. <br /> Refer to Section 3,Personal Protective Equipment Safe Work Practice for basic PPE requirements. <br /> 11.11 TRAINING REQUIREMENTS FOR ALL PERSONNEL INVOLVED IN CONFINED SPACE WORK <br /> Contractors working in confined spaces shall ensure workers have been trained to work in confined spaces.Training shall include <br /> but not be limited to: <br /> • Types and locations of confined spaces at the facility. <br /> • Chemical or physical hazards involved,including symptoms and consequences of exposure. <br /> • Work practices and techniques. <br /> • Atmospheric testing procedures. <br /> • Personal protective equipment,monitoring equipment,and ventilation equipment. <br />
