0 0 <br /> SAFE WORK PRACTICES FOR CONTRACTORS WORKING AT RETAIL PETROLEUMICONVENIENCE FACILITIES 37 <br /> Rescue procedures. <br /> Assigned duties. <br /> • Hands-on practice simulations of rescue procedures. <br /> 11.12 REFERENCES <br /> OSHA <br /> 29 CFR 1926.21 <br /> 29 CFR 1910.146 <br /> These may be obtained from OSHA7s web site at http://www.osha.govrindex.html) <br /> 12 Lockout-Tagout <br /> Minimum Required Knowledge <br /> 12.1 AFFECTED EMPLOYEE <br /> An affected employee is a worker whose job requires him/her to operate,use,or work near a machine or equipment being con- <br /> structed,serviced or maintained under lockout or tagout. <br /> 12.2 AUTHORIZED EMPLOYEE <br /> An authorized employee is a worker who locks out(or tags out)machines or equipment in order to perform servicing or mainte- <br /> nance on that machine or equipment.Examples include operations,maintenancelservice,construction or contractor workers who <br /> apply locks or tags to machinery or equipment and are authorized to do so because they are performing work on the equipment. <br /> 12.3 ENERGY ISOLATING DEVICE <br /> An energy isolating device is a mechanical device that physically prevents the transmission or release of energy,including but not <br /> limited to the following: <br /> • manually operated electrical circuit breaker, <br /> • disconnect switch, <br /> • line valve, <br /> • manually operated switch by which the conductors of a circuit can be disconnected from all ungrounded supply conductors <br /> and in addition,no pole can be operated independently(Only 110 volt switches meet these criteria). <br /> Push buttons,selector switches and other control circuit type devices are not energy isolating devices. <br /> 12.4 ENERGY SOURCES <br /> An energy source is any source of electrical,mechanical,potential,hydraulic,pneumatic,chemical,thermal,or any other Type of <br /> energy. <br /> 12.5 LOCKOUT AND TAGOUT DEVICES <br /> 12.5.1 Lockout Device <br /> A lockout device uses a positive means such as a separately keyed lock,locked chain,or cable to hold an energy isolating device <br /> in a safe position and prevent the energizing of a machine,equipment,or circuit.Lockout devices shall be strong enough to pre- <br /> vent removal without the use of excessive force(for example,using bolt cutters or other metal cutting tools). <br /> 12.5.2 Tagout Device <br /> A tagout device is a prominent warning device,such as a tag and a means of attachment,which can be securely fastened to an <br /> energy isolating device in accordance with an established procedure,to indicate that the energy isolating device and the equip- <br /> ment being controlled may not be operated until the tagout device is removed.Tagout devices and their means of attachment shall <br /> be strong enough to prevent accidental removal.Tagout device attachment shall be non-reusable,attachable by hand,self-locking, <br /> and non-releasable with a minimum unlocking strength 50 pounds such as a nylon cable tie. <br />
