• • <br /> SAFE WORK PRACTICES FOR CONTRACTORS WORKING AT RETAIL PETROLEUMICONVENIENCE FACILITIES 47 <br /> APPENDIX A.3—Lockout-Tagout—Section 12 <br /> A.3 Training and Program Audit <br /> A.3.1 LOCKOUT TAGOUT PROGRAM AUDIT <br /> Employers shall review their Lockout Tagout program annually and certify that the procedures are adequate to address the hazard, <br /> or note any deficiencies and take actions to correct them. <br /> A.3.2 TRAINING <br /> Employers shall provide training to ensure that workers understand the use of energy controls and have the skills to properly use <br /> them. The training shall include at least the following: Recognition of hazardous energy sources, Type and magnitude of the <br /> energy available in the workplace, <br /> • Methods and means necessary for energy isolation and control. <br /> • Purpose and use of the energy control procedure. <br /> • The prohibition of attempting to restart or reenergize machines or equipment which are locked out or tagged out. <br /> • The limitations of tags: <br /> i. Tags are essentially warning devices and do not provide physical restraint that is provided by a lock. <br /> ii. Tags are not to be removed without authorization of the person responsible for it. <br /> iii. Tags may never to be bypassed,ignored,or otherwise defeated. <br /> iv. Tags shall be legible and understandable by all authorized and, affected employees, and all other employees whose <br /> work operations are or may be in the area. <br /> v. Tags and their means of attachment shall be made of materials which will withstand the environmental conditions <br /> encountered in the workplace. <br /> ii. Tags may evoke a false sense of security,and their meaning and limitations need to be understood as part of the overall <br /> energy control program. <br /> iii. Tags shall be securely attached to energy isolating devices so that they cannot be inadvertently or accidentally detached <br /> during use. <br /> A.3.3 RETRAINING <br /> Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a <br /> change in machines,equipment or processes that present a new hazard,or when there is a change in the energy control proce- <br /> dures. <br />
