The designated operator failed to check and note the correct test dates for the secondary <br /> containment on the October 2013 through June 2014 designated operator monthly inspection reports. <br /> The correct test date should be 10-17-2014 and not the listed 10-29-2014 testing date. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> 205. The April 2014 designated operator monthly inspection report was not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit a copy to the EHD by July 30, 2014. <br /> 303. The 87-octane, 89-octane and 91-octane sump sensors were mounted to sump walls with screws <br /> and were located approximately 3-4 inches off the lowest point of the sump (up the saddle of the sump <br /> floor and not located to detect a leak at the earliest opportunity). The UDC 7/8 sensor was raised <br /> approximately 1/2 inch of the sump floor. Monitoring equipment shall be maintained to be able to detect a <br /> leak at the earliest possible opportunity. The technician relocated the sensors to the respective lowest <br /> points at the time of this inspection. CORRECTED. <br /> 307. During the inspection it was noted that the STP sump sensors in two STP sumps are attached to <br /> the sump walls with screws. All secondary containment for the UST system must be tight. Immediately <br /> evaluate the sumps to verify that these installed components will not jeopardize the sump integrity or <br /> interfere with the secondary containment testing. If these installed components jeopardize the sump <br /> integrity or interfere with the secondary containment testing have a properly licensed, trained, and <br /> certified contractor repair the altered components under permit and inspection of the EHD.
