ATTACHMENT TO POSDEF APPLICATION FOR <br /> UNDERGROUND STORAGE TANK CLOSURE PERMIT <br /> As discussed with Dennis Catanyag, of the San Joaquin County Public Health Services, <br /> Environmental Health Division ("PHS/EHD"), on 11 September 2002, the contractor that <br /> will abandon in-place the 1,000-gallon underground storage tank at the POSDEF facility <br /> has not yet been retained and the procedures for abandoning the tank have not yet been <br /> finalized with PHS/EHD. Discussion <br /> with Mr. Catanyag indicates that a representative from PHS/EHD must be present during <br /> the sampling and that fees for this oversight must be remitted before inspection of <br /> sampling activities can be conducted by PHS/EHD. For these reasons, POSDEF is <br /> submitting the tank closure application forms and a check in the amount of$400.50 for <br /> fees associated with expedited review before retaining a contractor to abandon the tank <br /> in-place. As agreed with Mr. Catanyag, POSDEF will submit the additional information <br /> (e.g., site health and safety plan, hazardous waste hauler, decontamination procedures) <br /> pertaining to the actual in-place abandonment of the tank when the information is <br /> obtained from the contractor retained by POSDEF. <br /> 12 September 2002 1 of 1 (EKI A20020.00) <br />
