ATTACHMENT TO POSDEF APPLICATION FOR UNDERGROUND STORAGE TANK CLOSURE PERMIT As discussed with Dennis Catanyag, of the San Joaquin County Public Health Services, Environmental Health Division ("PHS/EHD"), on 11 September 2002, the contractor that will abandon in-place the 1,000-gallon underground storage tank at the POSDEF facility has not yet been retained and the procedures for abandoning the tank have not yet been finalized with PHS/EHD. Box 7420 Fremont, California 94537-7420 Telephone: (800)499-3676 FAX: (510) 476-1786 General Engineering A/Haz License No. 733303 Sampling beneath the invert of the underground storage tank is planned for Thursday, 19 September 2002 and Underground Service Alert will be notified before the drilling takes place to identify any underground utilities that may exist in the vicinity. Discussion with Mr. Catanyag indicates that a representative from PHS/EHD must be present during the sampling and that fees for this oversight must be remitted before inspection of sampling activities can be conducted by PHS/EHD. For these reasons, POSDEF is submitting the tank closure application forms and a check in the amount of$400.50 for fees associated with expedited review before retaining a contractor to abandon the tank in-place. As agreed with Mr. Catanyag, POSDEF will submit the additional information (e.g., site health and safety plan, hazardous waste hauler, decontamination procedures) pertaining to the actual in-place abandonment of the tank when the information is obtained from the contractor retained by POSDEF. 12 September 2002 1 of 1 (EKI A20020.00)
