The facility and USTs are abandoned. No one is <br /> on site. Unable to verify if the UST monitoring system is operational. No access to the property is <br /> available. <br /> Owner Mr. Rai of Petros Rai Stations LLC notified the San Joaquin County Environmental Health <br /> Department (EHD) on approximately February 1, 2012 that the facility was "given back" to BBCN Bank. <br /> The EHD verified that the facility was closed on February 21, 2012. A Notice to Abate to restore UST <br /> monitoring and pay the operating permit fees was sent to Mr. Rai and BBCN Bank on March 28, 2012. <br /> Violations <br /> 101. A permit to operate the UST system has not been issued. No person may own or operate an UST <br /> unless a permit for its operation has been issued by the local agency to the owner or operator of the <br /> UST system. Immediately obtain a permit to operate a UST system from the EHD. An "Operating <br /> without a Permit" penalty applies. <br /> 102. Annual operating permit fees for the current year have not been paid. Owner and/or operator of <br /> the UST system shall pay fees established by each certified unified program agency's single fee system. <br /> Immediately pay current and any outstanding fees from past non-payment at the, EHD. <br /> 104. Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Immediately complete and submit a <br /> copy of the certificate of financial responsibility and associated documents. <br /> 105. UST facility and tank forms on file with the EHD are not current. Any change of information on <br /> these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and <br /> submit a copy of the facility and tank forms to the EHD.
