The monitoring and response plans are not current and not approved by the EHD. The monitoring <br /> and response plans must be current and approved by the EHD. Complete and submit a copy of the <br /> monitoring and response plans for approval by the EHD. An approved copy will be returned for your <br /> records. <br /> 109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on June 6, 2011 and was due by June 29, 2012. Testing has not been completed <br /> and is past due. These tests are required once every 12 months. Immediately schedule these tests and <br /> provide 48 hours notification to the EHD. Please be aware that follow up inspections will be charged at <br /> our current hourly rate. <br /> 119, 120 & 317. This UST system was found to be illegally abandoned. No person shall abandon an <br /> UST system without complying with the requirements of HSC 25298. Immediately obtain a permit from <br /> the EHD to comply with HSC 25298. A "Failure to Obtain a Permit" penalty applies. <br /> 201. Since the facility is closed I was unable to determine if maintenance and monitoring records are <br /> retained on site. These records shall be kept on site for at least 3 years . Immediately locate and <br /> maintain these records on site and submit copies to the EHD. <br /> 202. Since the facility is closed I was unable to determine if a copy of the UST operating permit and the <br /> monitoring and response plans are retained on site. A copy of these documents shall be retained at the <br /> facility. Immediately obtain a copy of these documents and retain it on site. <br /> 204. Since the facility is closed I was unable to determine if the designated operator documented all the <br /> alarms, checked the correct test dates on the designated operator monthly inspection report, visually <br /> inspected the UST system for leaks and verified all employees have been trained. <br /> 205. Since the facility was closed I was unable to determine if the designated operator monthly <br /> inspection reports were retained on site since the last EHD routine inspection on June 6, 2011. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Submit copies to the EHD. <br /> 206. Since the facility was closed I was unable to determine if the designated operator employee training <br /> was performed. The designated operator shall train facility employees for which he or she is responsible <br /> in the proper operation and maintenance of the UST system once every 12 months.
