The <br /> missing alarms include: 1-7: 87 sump STP sump fuel alarm on April 21, 2017. <br /> Except for the 6/29/16 designated operator(DO) monthly inspection report, the DO failed to print out the dates on <br /> the associated panel printout for May 2016 through April 2017. Also the DO listed dispenser 11-12 on all of the <br /> monthly inspection reports and there is no dispenser 11-12 identified at this facility. <br /> The DO did not print out the alarm histories from the Veeder Root panel for the DO monthly inspections as follows: <br /> L1 through L12 for the 6/29/16 report; <br /> L12 for the 7/28/16 report; <br /> L1 for the 8/26/16 report; <br /> L1 through L12 for the 11/29/16 report; <br /> 1-5, L9 through L12 for the 12/29/16 report; <br /> No panel printout for 1/27/17 report; and <br /> 1-4, L9 and L10 for the 2/24/17 report,- <br /> There <br /> eport;There was no signature on the 4/23/17 DO report. <br /> The DO did not address the T1:UNL 87 overfill alarms as follows: <br /> June 22, 23, and 25, 2016 on the 6/29/16 DO report; (also the High Product alarms on June 8 and 19, 2016) <br /> July 12, 13, and 21, 2016 on the 7/28/16 DO report; <br /> July 29,August 6 and 19, 2016 on the 8/26/16 DO report; <br /> September 15, 28 and 29, 2016 on the 9/30/16 DO report; <br /> December 23, 25, and 29, 2016 on the 12/29/16 DO report; (and the High Product alarm on December 23, <br /> 2016.) <br /> February 16, 22, and 23, 2017 on the 2/24/17 DO report; (and the High Product alarms on February 11 and 23, <br /> 2017) <br /> April 21 and 23, 2017 on the 4/23/17 DO report; (and the High Product alarm on April 19, 2017) <br /> March 28 and 30, 2017 on the 3/31/17 DO report; (and the High Product alarm 3/3/2017) <br /> The DO did not address the T2:Premium UNL overfill alarms as follows: <br /> February 20, 2017 on the 2/24/17 report; and <br /> March 8, 2017 on the 3/31/17 report. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report.
