Add the line leak detector locations to the site plan and <br /> legend and add the word "sensor"to the word "Annular" in the legend. The site plan must be submitted identifying <br /> the locations where monitoring will be performed. Immediately log into the California Environmental Reporting <br /> System(CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> The facility failed to document and address the L7:87 sump STP sump fuel alarm on April 21, 2017. Going forth, <br /> make sure that all alarms are documented and addressed. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the April 23, 2017 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: L7: 87 sump STP sump fuel alarm on April 21, 2017. <br /> Except for the 6/29/16 designated operator (DO) monthly inspection report, the DO failed to print out the dates on <br /> the associated panel printout for May 2016 through April 2017. Also the DO listed dispenser 11-12 on all of the <br /> monthly inspection reports and there is not dispenser 11-12. <br /> The DO did not address the T1:UNL 87 overfill alarms as follows: on April 21 and 23, 2017 on the 4/23/17 DO <br /> report; on March 28 and 30, 2017 on the 3/31/17 DO report. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The diesel submersible turbine pump (STP) sump 205 sensor was leaning over and not located at the lowest point <br /> of the sump and not located to detect a leak at the earliest opportunity. Also the under dispenser containment(UDC) <br /> 9-10 208 sensor was resting on aboard and not located at he bottom of the UDC. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest <br /> point of the diesel STP sump and UDC 9-10 sump by the service technician and the board was removed from UDC <br /> 9-10.
