• Stopping work as required to ensure personal safety <br /> and protection of property or where noncompliance with <br /> safety requirements are found <br /> iDetermiing and posting routes to capable medical <br /> facilities and emergency telephone numbers (including <br /> poison control centers) , and arranging for emergency <br /> transportation to medical facilities <br /> • Examining work party members for symptoms of exposure <br /> 1 or stress <br /> • Ensuring that each team member has been given the <br /> proper medical clearance by a qualified medical <br /> consultant; monitor all team members to determine <br /> compliance with the applicable physical requirements <br /> as stipulated in the H&S plan. <br /> Technicians/Subcontractors <br /> Technicians, subcontractors, and other personnel on site will be <br /> responsible for understanding and complying with all site <br /> requirements . H&S requirements will be included in subcontracts. <br /> On site or preproject training will be conducted, and topics <br /> cited in Section 1 .4 .2 will be discussed. All participants will <br /> sign a written statement indicating they are aware of the site <br /> hazards and protective requirements . <br /> MEDICAL SURVEILLANCE <br /> Physical Examination <br /> As required by IT Policies and Procedures 9410 . 10 and 9410 . 2B, <br /> all IT personnel on site will have successfully completed a <br /> preplacement or periodic physical examination as appropriate . <br /> This examination has been designed to comply with OSHA <br /> ' requirements in 29 CFR 1910 . 120 for hazardous waste site <br /> operations . <br /> iThe IT medical surveillance program examination consists of: <br /> 1 Medical and occupational history form (detailed <br /> questionnaire for new employees, short questionnaire <br /> for periodic exams) <br /> • Physical examination <br /> A-10 <br /> MA:ENG:9262-AppA/041291 <br />
