0 <br /> • Complete blood count with differential <br /> • SMAC 23 <br /> jUrinalysis (dipstick and microscopic) <br /> • Chest x-ray <br /> • Pulmonary function test (FEV/FVC) <br /> • Audiometric examination <br /> • Electrocardiogram for persons older than 45 years of <br /> age, or if medically indicated <br /> • Drug and alcohol screening <br /> • Visual acuity. <br /> IThe following information is provided to the examining physician: <br /> • Description of employee' s duties <br /> • Anticipated exposure levels <br /> • Description of the PPE to be used <br /> • Information from previous medical exams . <br /> IA copy of the medical examination is provided the employee at <br /> his/her request . The employee shall be informed of any medical <br /> conditions that would result in work restrictions or that would <br /> preclude him/her from working at hazardous waste sites. <br /> All subcontractor personnel who have potential for exposure to <br /> hazardous materials shall have successfully completed an <br /> examination similar to the preplacement physical . The cost for <br /> medical surveillance will be paid by the subcontractor. All <br /> physicals will be approved by an occupational physician. The <br /> Field Supervisor will verify medical surveillance documentation. <br /> Medical Records <br /> Medical records will be maintained in accordance with the <br /> requirements of 29 CFR 1910 . 120 and shall be kept for 30 years . <br /> Employee confidentiality shall be maintained. <br /> A-11 <br /> ' MA:ENG:9262-AppA/041291 <br />
