Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by December 5, 2010. <br /> 7. Maintenance records and service receipts for the following alarms were not found on site: S4 - 91 <br /> product vacuum sensor, communication alarm, May 11, 10, March 14, 10, January 1, 2010; L26 -diesel <br /> fill sump interstitial sensor, fuel alarm, December 3, 2009; L-2 - Dispenser 1/2 interstitial sensor fuel <br /> alarm, Sept 22, 2010. These records shall be kept on site for at least 3 years. Immediately locate and <br /> maintain the maintenance records and service receipts on site and submit copies to the EHD by <br /> December 5, 2010. <br /> 22. Dispenser 10 and 12 contained diesel on November 5, 2010 and the releases were not recorded. <br /> Any release from the primary containment that does not escape the secondary containment and was <br /> cleaned up within eight hours of being detected shall be recorded with the following information: <br /> 1. Operators name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. Inspector provided form.
