The designated operator failed to document all the alarms for the previous month and check that <br /> they were responded to appropriately on the November 2009, December 2009 and September 2010 <br /> designated operator monthly inspection reports. The missing alarms include: S4 - 91 product vacuum <br /> sensor, communication alarm, May 11, 10, March 14, 10, January 1, 2010; L26 -diesel fill sump <br /> interstitial sensor, fuel alarm, December 3, 2009; L-2 - Dispenser 1/2 interstitial sensor fuel alarm, <br /> September 22, 2010. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, <br /> and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> 47. The 87 product fill sump interstitial sensor failed when tested. The sensors monitoring the liquid <br /> level in the interstitial space shall be maintained so that a breach in the primary or secondary <br /> containment is detected before the hazardous substance is released into the environment. Correct <br /> immediately by having a properly licensed, trained, and certified contractor replace the failed component <br /> with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day <br /> from the EHD. If the failed component can't be replaced immediately, there is a possibility that the 87 <br /> UST system may be red tagged to prevent fuel inputs. <br /> Notes <br /> 1) Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by December 5, 2010. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAAJNG TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE(11122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector.
