The designated operator also failed to document all the alarms from the attached alarm <br /> history on the December 2014 through March 2015 and September 2015 through November 2015 designated operator <br /> monthly inspection reports and failed to check that they were responded to appropriately. The missing alarms include: <br /> -L2 sensor out alarm on July 20, 2015 <br /> - L12 fuel alarm on September 22, 2015, September 15, 2015 and September 7, 2015 <br /> -L16 fuel alarm on November 9, 2015 <br /> -L19 fuel alarm on February 9, 2015 <br /> -L20 fuel alarm on December 2, 2014 <br /> -L21 fuel alarm on December 23, 2014, December 15, 2014 and December 2, 2014 <br /> -L22 fuel alarm on December 23, 2014 <br /> -L26 fuel alarm on December 13, 2014 <br /> -Q1 gross line fail on November 19, 2015 <br /> -Q2 gross line fail on March 2, 2015 and February 27, 2015 <br /> -Q3 gross line fail on January 2, 2015 and December 5, 2014 <br /> -S5 no vacuum on December 15, 2014 <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training roster was incomplete. The designated operator shall train facility <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST system once every <br /> 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
