The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHID for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cindy Vo at(209) 468-8257 to arrange the following inspection appointments (a 48-hour <br /> notice isrequired for inspection appointnoento). <br /> a. Secondary containment testing onall tanks and sumps after backfill. <br /> h. Verification ufall fill drop tube measurements. <br /> Please huaware that the facility cannot operate the UST system prior tothe EHDinspection and <br /> approval. <br /> 4. Secondary containment testing for all sumps are required six months after installation. Provide <br /> 40hours notification tothe EHDwhen this test inscheduled. <br /> 5 Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically present <br /> at the work site who possesses a valid, current California LIST Service Technician certificate <br /> issued bythe International Code Council (|CC), and <br /> o. Possesses a valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, orserviced. <br /> 0. Individuals installing underground storage tank system components must possess or work under <br /> the direct and personal supervision of an individual physically present at the work site who <br /> possesses all ofthe following: <br />
