In all situations, when on site emergency results in evacuation of the Exclusion <br /> Zone, personnel shall not reenter until: <br /> 1. The conditions resulting in the emergency have been corrected. <br /> 2. The hazards have been reassessed. <br /> 3. The HASP has been reviewed and amended, if necessary. <br /> 4. Site personnel have been briefed on any changes in the HASP. <br /> ' 10.0 MEDICAL SURVEILLANCE AND TRAINING REQUIREMENTS <br /> 10.1 Medical Surveillance Requirements <br /> ' An essential requirement of the HASP mandates that project personnel, who may be <br /> exposed to materials having potentially adverse and deleterious health effects, be <br /> ' medically certified and adequately trained prior to beginning work at the site. Kroeker <br /> corporate medical surveillance programs have been implemented to establish a medical <br /> baseline and periodic monitoring for any symptoms of overexposure for individuals who <br /> ' participate in remedial investigations, feasibility studies, and construction phase services <br /> at hazardous waste management sites. Additionally, the program is intended to <br /> determine the individual's capability for performing on-site work, including the use of <br /> ' respiratory protective equipment. <br /> Each individual receives a physical examination by a licensed physician. Standard <br /> ' parameters such as height, weight, vision, temperature, and blood pressure and noted. <br /> The complete physical examination is complemented by occupational and medical; <br /> ' histories. Other information obtained with the medical examination includes: chest x- <br /> rays, electrocardiogram, pulmonary function test, urinalysis, and blood tests. <br /> Attachment A describes the medical surveillance requirements for the Kroeker employees <br /> ' working on this site. <br /> Subcontractor personnel will be required, at a minimum, to meet all applicable medical <br /> monitoring requirements identified in OSHA's General Industry Standards (29 CFR 1910) <br /> and Construction Standards (29 CFR 1926), including the requirements in 29 CFR <br /> 1910.120.
