A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 211 CCR 2715(f)Designated operator employee training not performed or log not kept. <br /> The designated operator employee training did not contain date of hire for employees hired after july 1, 2005. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1)Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo chain and float in the 5/6, 1/2 and 7/8 UDC sumps failed to stop the flow of product at the dispenser <br /> when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br /> of product at the dispenser when it detects a leak. Technician adjusted the chains, retested and passed. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 product Submersible Turbine Pump(STP)sump. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service <br /> technician removed about one cup of liquid from the 87 product STP sump.
