An accurate UST Monitoring Site Plan was not submitted. The site plan is lacking the approximate location of the underground storage tank piping.A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a revised copy of the UST Monitoring Site Plan. This is a repeat violation,Class Il. 110 CCR 2637(e)Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on 6/20/2017 and a test report was submitted on 9/19/2017, 3 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 111 CCR 2638(a) Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification(MSC)and leak detector testing were performed on February 14, 2016, more than two months past the due date of November 2015.The annual MSC and leak detector testing should have been completed in November 2016 and November 2017. Instead the MSC and leak detector testing were completed in February 2017 and today(February 13, 2018).Testing is more than two months past due.These tests are required once every 12 months in November.Take all necessary precautions to ensure testing is performed in a timely manner. This is a repeat violation, Class ll. 112 HSC 25284.2 Failed to test spill containers annually. Annual spill container testing was performed on February 14,2016, more than two months past the due date of November 2015.The annual spill container testing should have been completed in November 2016 and November 2017. Instead the spill container testing was completed in February 2017 and today(February 13,2018).Testing is more than two months past due.These tests are required once every 12 months in November.Take all necessary precautions to ensure testing is performed in a timely manner.
