San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ARCH ARCO AM PM" 4855 S HWY 99 E FRONTAGE RD, STOCKTON February 13, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS it,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site.The facility alar logs <br /> documented the C1 Fuel Alar on 11/16/17, but did not document how the alar was addressed.The facility alar <br /> logs did not document nor address the L11 87 STP Sensor Out alar on 10113/2017, L17 91 STP Fuel alar on <br /> 711/17, and the L13 89 Fill Sensor Out and Fuel alarms on 5/30/17.These records shall be maintained on site for at <br /> least three years. Monitoring records include:(1)date and time of all monitoring or sampling; (2)monitoring <br /> equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis <br /> performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water <br /> elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately ensure that the <br /> facility is documenting and addressing all alarms, and that the maintenance and monitoring records for the last three <br /> years are on site. Provide a statement and any supporting documentation to the EHD stating how each of the <br /> above-mentioned alarms were addressed. <br /> This is a repeat violation, Class ll. <br /> 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator(DO)failed to check and note the correct test dates for the annual monitoring system <br /> certification on the 2/24/2017 designated operator monthly inspection report.The DO incorrectly noted the MSC was <br /> performed on 2/24/17,when the MSC was performed on 2/14/17. During the monthly inspection,the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed and <br /> document the dates they were done. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> The designated operator failed to document all the alarms from the attached alarm history on the 6/13/2017 and <br /> 7/28/17 designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: L13 89 Fill Sensor Out and Fuel alarms on 5/30/17, L17 91 STP Fuel <br /> alarm on 7/1/17. During the monthly inspection,the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 302 HSC 25291(b)UST system incapable of detecting leak from primary into secondary containment(pre-Jul 2003). <br /> The Schrader valve in the 89-product submersible turbine pump(STP)sump was facing up, and it was unverifiable <br /> if this Schrader valve was open due to the depth of the 89-product STP sump.Any potential release from the <br /> primary containment into the secondary containment in the 89-product piping does not flow into a collection sump. <br /> All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the <br /> low point of the underground piping.
