Line B on the form <br /> is blank. It lists that the CFO letter is covering $500,000 per occurrence, but the CFO letter submitted is covering <br /> $5,000. The annual aggregate amount checked is$2,000,000—please verify that this is the correct amount for the <br /> number of tanks owned. <br /> Current financial responsibility documents are required to be submitted annually. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 104 CCR 2711 UST Operating Permit Application for Facility and Tank information not submitted or current. <br /> UST Tank Information forms are not current in CERS. The Tank forms incorrectly indicate that the site does not have <br /> fill sumps. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into <br /> CERS, update the required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. The monitoring plan does not include the change <br /> of the 87 leak detector. The monitoring plan must be uploaded to the California Environmental Reporting System <br /> (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The Monitoring Site Plan submitted on CERS does not <br /> show the locations of the sensors or the monitoring panel. A site plan must be submitted identifying the locations <br /> where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a Class II violation. <br /> 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br /> Secondary containment testing was performed on December 3, 2014 and a test report for the UDC testing was <br /> submitted on January 30, 2015, one month late. A copy of the test report must be submitted within 30 days of the <br /> tests.
