These tests are required once every 12 months. Take all <br /> necessary precautions to ensure testing is performed in a timely manner. <br /> This is a Class II violation. <br /> 112 HSC 25284.2 Failed to test spill containers annually. <br /> Annual spill container testing was last performed on November 25, 2014 and was completed on February 8, 2016, <br /> three months past due. This test is required once every 12 months. Take all necessary precautions to ensure testing <br /> is performed in a timely manner. <br /> This is a Class II violation. <br /> 115 CCR 2715(a) Failed to inform the EHD of a change of designated operator within 30 days. <br /> A new designated operator(Affordatest- Dave Winkler,Zane Nimmo, Ed Stearns, Felix Ramirez)was hired in October <br /> 2015 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the <br /> EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> This is a Class II violation. <br /> 123 CCR 2661 Permit to repair UST system not obtained. <br /> A cold start of the monitoring panel was performed on March 31, 2015. A permit approval letter was issued April 6, <br /> 2015,with a condition stating"inspections for emergency work must be scheduled within five days of permit approval." <br /> A sensor functionality inspection was not scheduled or performed. Emergency repairs relating to UST system <br /> monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant <br /> to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a <br /> permit submitted to and approved by the EHD prior to beginning work. The monitoring system was verified functional <br /> at today's monitoring system certification. Ensure that all permit conditions are followed for all future repair permits. <br /> This is a Class II violation. <br /> Page 5 of 8 <br />
