The annual MSC and leak detector testing should <br /> have been completed in November 2016. Instead the MSC and leak detector testing were completed today <br /> (February 14, 2017). Testing is more than two months past due. These tests are required once every 12 months <br /> in November. Take all necessary precautions to ensure testing is performed in a timely manner. <br /> This is a repeat violation, Class II. <br /> 112 HSC 25284.2 Failed to test spill containers annually. <br /> Annual spill container testing was last performed on February 8, 2016, more than two months past the due date of <br /> November 2015. The annual spill container testing should have been completed in November 2016. Instead the <br /> spill container testing was completed today (February 14, 2017). Testing is more than two months past due. <br /> These tests are required once every 12 months in November. Take all necessary precautions to ensure testing is <br /> performed in a timely manner. <br /> This is a repeat violation, Class II. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing were performed on February <br /> 8, 2016 and a test report was submitted on June 6, 2016 , approximately three months late. A copy of the test <br /> report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and <br /> submittal of test reports are performed in a timely manner. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The Annual Monitoring System <br /> Certification testing report for November 25, 2014 was not found on site during the inspection. The facility failed to <br /> document and address the 1-1:Dispenser 5-6 Dispenser pan sensor out alarm, and fuel alarm on 1/22/17; and the <br /> Cl: 87 Fill sump piping sump fuel alarms on 7/1/16 and 10/18/16. These records shall be maintained on site for at <br /> least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring <br /> equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6) results of inventory readings and reconciliations.
