According to the alarm history <br /> and the designated operator monthly inspection reports, a C1 (87 fill)fuel alarm occurred on 8/11/15. The alarm was <br /> still present on 8/27/15. No maintenance records were found on site for the follow up and corrective action taken in <br /> response to this alarm. The September 2015 designated operator inspection report did not note that this alarm was <br /> still active. These records shall be maintained on site for at least three years. Monitoring records include: (1) date <br /> and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of <br /> any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges <br /> or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies of missing records to the EHD. <br /> This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the correct test dates for the last monitoring system certification on <br /> the November 2015 through January 2016 designated operator monthly inspection report. They list the date as <br /> 3/31/15, the date of the cold start. The designated operator failed to check and note the correct test dates for the last <br /> secondary containment test on the August, September, and October 2015 designated operator monthly inspection <br /> reports. The October report lists the date as 6/2014. The August and September 2015 inspection reports do not list a <br /> date. During the monthly inspection, the designated operator shall check that all required testing and.maintenance for <br /> the UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to document all the alarms from the attached alarm history on the December 2015 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: T1 (87) 11/27/15 overfill alarm. During the monthly inspection, the designated operator shall <br /> review the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a Class II violation. <br /> Page 6 of 8 <br />
