The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Contact Betty Ho at (209) 468-3455 to arrange the following inspection appointments (a 48- <br />hour notice is required for inspection appointments). Please be aware that inspections for <br />emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br />monitoring has been restored to meet the requirements of Title 23 California Code of <br />Regulations and the Health & Safety Code. A copy of the alarm history shall be printed <br />prior to performing the "cold start" of the Veeder Root system and made available at the <br />time of EHD inspection. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, <br />current contractor's licenses issued by the Contractors State License Board (may be issued <br />to the installation company), or a Tank Tester's license issued to the individual by the State <br />Water Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service Technician <br />certificate issued by the International Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />1868 E.
