Page 3 of the Application for Underground Storage Tank Installation Permit does not specify <br /> a contractor. Specify a contractor and submit a copy of the contractor's technician <br /> certifications. Submit the updated Page 3 of the Application for Underground Storage Tank <br /> Installation Permit. <br /> Response: Jones Covey Group, Inc. has been selected as the contractor. Enclosed are <br /> copies of the contractor's technician certifications, as well as the updated Page 3 of the <br /> application. <br /> 2. Page 4 of the Application for Underground Storage Tank Installation Permit indicates that <br /> Enhanced Leak Detection (ELD) must be performed prior to the final inspection. A separate <br /> permit application is required to install ELD.
