Page 7 section G of the Application for Underground Storage Tank Installation Permit does not list the manufacturer and model of the overspill containment having minimum capacity of 5 gallons provided at each tank fill location. Fill in the blank. Submit corrected version. Response: The spill bucket information has been included on the revised application included with this resubmittal. The Manufacturer is: OPW and the Model No. is P511- EVRDV. 5. Page 7 section G of the Application for Underground Storage Tank Installation Permit lists an OPW 61fSTOP-2000 as the overfill prevention at 95%. However, an OPW61fSTOP is approved for aboveground storage tanks. Choose equipment that is approved for underground storage tanks. Correct the discrepancies. Submit corrected version. Response: Due to the fact that the underground fuel additive tank receives a pressurized delivery from the tanker truck, the only suitable full drop tube is the 61fSTOP-2000. Although typically used for ASTs, the function has been reviewed, revised and approved by Containment Solutions and OPW. See enclosed document "Pressurized Product Deliveries to CSI Underground Tanks" prepared by Containment Solutions dated May 12, 2011 is included in this resubmittal. 6. Page 7 section G of the Application for Underground Storage Tank Installation Permit indicates that an approved overfill prevention device must be provided at each tank fill location. Page 7 section H of the Application for Underground Storage Tank Installation Permit does not have checked Tie-downs, when used, constructed of non-corrosive material or coated steel. Check it. Page 1 of the 8-9-12 monitoring plan in section 490-33 does not list the sump and sump interstitial sensors manufacturer and model numbers. Update it. Submit corrected version. Response: Page 1, Section 490-33 has been revised and is included with this resubmittal. The model numbers are: 794380-208 and 304. 9. Page 2 of the 8-9-12 monitoring plan in section 490-71 lacks the description of audible visual overfill alarm set @ 90% tank capacity. Update it. Submit corrected version. Response: The description has been revised.
